About the Role
The Quality Improvement Specialist (QIS) is responsible for all quality activities, including monitoring, implementing, and evaluating program processes and clinical outcomes to meet Health Home Policies and Procedures. This role involves training new staff, ensuring documentation adherence, and monitoring Core Services and outreach initiatives in support of Samaritan Daytop Village's mission.
Responsibilities
- Implement and monitor the organization’s quality improvement program.
- Ensure processes are in place to monitor and measure performance improvement and services rendered.
- Provide monthly, quarterly, and annual Performance Improvement reports and trend analysis to the Program Director.
- Direct and coordinate quality improvement activities using continuous quality improvement principles and methodologies.
- Ensure processes to monitor and evaluate the coordination of care provided by workers.
- Provide guidance for Outreach Staff through training, perform compliance reviews, ensure regulatory requirements are met, and monitor documentation.
- Monitor the timely completion of assignments, Health Risk Assessments, and Care Plans.
- Confirm daily submission of entries.
- Collaborate with the billing department to resolve account, claim denial, and charge capture issues.
Requirements
- Associates Degree in Psychology, Human Services, Social Work, or a related field.
- 3-5 years of job-related experience in Process Improvement and Continuous Quality Improvement, preferably within a Health Home/Care Coordination program.
- Computer literacy including proficiency in Microsoft Office Suite and EHR.
- Excellent interpersonal, communication, analytical, and organizational skills.
- Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
- Ability to work independently and as part of a team.
- Familiarity in developing reports.
