About the Role
The Healthcare Transformation Project Lead provides regional leadership and oversight for the implementation deliverables under the New York State Rural Health Transformation Program (RHTP). This role serves as the operational lead responsible for translating strategic program goals into coordinated, executed activities across a 10-county region. The Project Lead oversees regional budget, workforce development, clinical and IT workflow changes, and data infrastructure activities; ensures alignment among health system project managers, hospital partners, primary care practices, and community-based organizations and reports, program progress, and risks across the RHTP initiatives. This position acts as the primary point of accountability for ensuring all deliverables are implemented on time, within budget, and in compliance with program requirements.
Responsibilities
- Develop and maintain detailed implementation timelines for all objectives, aligning activities across partners and sub-regions.
- Coordinate cross-functional work between clinical, operational, and administrative teams, proactively identifying dependencies and sequencing risks.
- Ensure deliverable milestones are tracked and met, adjusting plans as program priorities or timelines shift.
- Build and sustain productive working relationships with hospital partners, primary care practices, CBOs, county health departments, and other community-based organizations.
- Serve as a primary liaison to align diverse stakeholder priorities with program goals, facilitate regular engagement touchpoints, and identify and resolve barriers to partner participation and implementation progress.
- Oversee workforce planning across sub-regions, including recruitment strategy, hiring pipelines, onboarding processes, and ongoing deployment of staff.
- Assess staffing needs and FTE coverage against program demands, working with Sub-Region PMOs to address gaps, rebalance resources, and ensure equitable service capacity.
- Direct the design, rollout, and standardization of protocols for screening, referral, navigation, and care coordination across all sub-regions.
- Ensure partner organizations are trained and supported in adopting workflows consistently, and establish mechanisms to monitor adoption, identify variation, and drive continuous process improvement.
- Establish and oversee systems for tracking deliverable progress, encounter data, and outcome measures across the program.
- Work closely with the Performance Analyst and BI team to build out data infrastructure, guide dashboard development and data quality, synthesize performance trends, and translate findings into clear reports and recommendations.
- Proactively identify implementation risks, operational bottlenecks, and systemic barriers across sub-regions before they threaten deliverable timelines.
- Develop and implement mitigation strategies and exercise judgment on when issues require escalation to leadership.
- Monitor regional program budgets across all deliverables, tracking spending against plan and flagging variances early.
- Anticipate the need for budget modifications as program priorities, staffing needs, or deliverable scope evolve, and lead preparation of documentation and justification for timely approval.
- Lead efforts to assess the long-term viability of program activities beyond the initial funding period, including analysis of Value-Based Payment (VBP) opportunities and alternative revenue models.
- Develop transition plans and funding diversification strategies in partnership with leadership and sub-region stakeholders to support continuity of services at scale.
- Lead the identification, planning, and coordination of IT and clinical system workflow changes required to operationalize program deliverables across partner sites.
- Collaborate closely with IT and EPIC subject matter experts to translate program requirements into system configuration, build, and workflow modifications.
- Ensure changes are sequenced appropriately and aligned with implementation timelines.
- Serve as a bridge between program, operational, and technical teams to resolve workflow gaps and support consistent adoption across sub-regions.
- May assume additional responsibilities as requested.
Requirements
- Bachelor's degree in Project Management, Public Health, Health Administration, Business Administration, or a related field.
- Minimum of 4–5 years of experience in health care, community health, or program coordination, preferably in a rural health or regional network setting.
- At least 2–3 years managing multi-site or regional initiatives highly preferred.
- Demonstrated experience in stakeholder engagement, partner management, and facilitation of multi-organizational workgroups.
- Strong organizational and project management skills, with the ability to manage multiple deliverables and deadlines across sub-regions.
- Experience coordinating data collection activities across diverse data sources, tracking deliverable-based performance metrics, and preparing programmatic and financial reports.
- Strong knowledge of performance dashboards and outcome reporting requirements in a grant-funded or government-contracted environment.
- Excellent written and verbal communication skills, with the ability to translate complex program requirements for diverse audiences.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Teams).
- Ability to travel within the sub-regions for partner and network meetings.
