About the Role
As a member of a primary health care (PHC) Diabetic education Centre team, the PHC Dietitian provides a range of PHC services including health promotion, illness prevention, screening, treatment, and monitoring of people living with diabetes. Expertise may also be required to support people living with other chronic health problems, community development, and advocacy. The incumbent will collaborate with others to increase community capacity as an educator, researcher, and clinical practitioner within a holistic, client/family-centred care framework.
Responsibilities
- Provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management.
- Provide health assessment, interventions, and appropriate follow-up in accordance with acts, regulations, policies, standards, guidelines, mission, and objectives of relevant professional bodies and health authorities.
- Collaborate with others to increase community capacity as an educator, researcher, and clinical practitioner.
- Provide services within a holistic, client/family-centred care framework.
- Conduct health promotion, illness prevention, screening, treatment, and monitoring of people living with diabetes.
- Support people living with other chronic health problems.
- Engage in community development and advocacy.
Requirements
- Bachelor of Science in Human Nutrition at a recognized university.
- Dietetic internship (a 1 year integrated or post graduate program).
- Current Membership in Nova Scotia Dietetic Association required.
- Annual continuing education to maintain Nova Scotia license.
- Current CPR/BLS certification is required.
- Demonstrated time management and organizational skills.
- Familiarity with principles of Motivational Interviewing.
- Ability to work collaboratively with other disciplines and committees to achieve excellence in patient care.
- Computer proficiency in MS Office (Word, PowerPoint), email and internet.
- Ability to work well independently as well as within a multidisciplinary team environment.
- Excellent verbal and written communication skills.
- Resume includes all relevant education, experience, training, and certifications.
