About the Role
MJHS is seeking a dedicated professional to provide psychosocial support and Social Work services to Hospice patients and their family members. This role is crucial in supporting individuals and families navigating life-limiting illnesses and end-of-life care within various settings.
Responsibilities
- Provide psychosocial support and Social Work services to Hospice patients and their family members in inpatient units, nursing homes, or home care settings.
- Offer pre and post bereavement services.
- Serve as a social work member of the hospice Interdisciplinary Group (IDG) and act as a liaison between hospice, inpatient, and nursing home staff.
- Develop the social work plan of care, set goals, and make recommendations based on patient and family needs.
- Maintain accurate and current documentation of palliative social work assessments and interventions.
- Participate in discharge planning for patients admitted to inpatient facilities.
Requirements
- Graduate from an accredited School of Social Work.
- Minimum of one year of experience in Social Work within a healthcare setting.
- Knowledge of Word and Excel preferred.
- New York State MSW licensure.
- Valid NYS Driver's license preferred.
- Ability to work within an interdisciplinary team environment.
- Excellent organizational, communication, and listening skills.
- Ability to input clinical data proficiently.
