About the Role

This role involves supporting residents in various aspects of their daily lives, including medical appointments, medication management, personal care, and communication.

Responsibilities

  • Schedule and attend medical, dental, and lab appointments, ensuring coverage from other staff when necessary.
  • Notify parents/clinicians and day programs of upcoming appointments.
  • Maintain adequate supplies of medications and non-prescription items, storing them appropriately and discarding expired items.
  • Pick up medications and supplies as needed.
  • Check medication bottles and bubble packs, reporting any errors to the RN.
  • Assign medication-certified staff (AMAP) for each shift, ensuring at least one is available.
  • Assure AMAPs administer medications in accordance with certification requirements.
  • Teach CPR-SFA, train staff as scheduled by the nurse, and maintain staff records.
  • Check and restock first aid kits in vans and residences.
  • Verify consumer-specific medication information sheets and ready-to-go packets.
  • Ensure vital signs and heights/weights are obtained as ordered.
  • Facilitate improved gross motor, fine motor, and range of motion skills.
  • Promote and facilitate self-care and personal hygiene.
  • Assist residents with bathing, toileting, dressing, and eating, including modeling appropriate behavior and using therapeutic equipment.
  • Perform housekeeping tasks such as meal preparation, cleaning, laundry, and shopping when needed.
  • Foster receptive and expressive communication through various methods, including sign language, pictograms, and electronic devices.
  • Accurately and consistently complete all required documentation using PrecisionCare software and written forms.
  • Attend and actively participate in staff meetings, case reviews, and in-service sessions.
  • Implement fire, safety, and emergency procedures, assisting with fire drills.
  • Administer medications to residents according to policy.
  • Accurately and consistently implement behavior support plans/SCIP-r.
  • Safely drive agency vehicles and assist with transportation for recreational activities and appointments.
  • Assume shift supervisor responsibilities when requested.
  • Serve as an advocate for residents, including writing reports, data collection, case reviews, and classroom/day program visits.
  • Perform other duties as assigned by the manager.

Requirements

  • Must be at least 18 years of age.
  • Ability to read, write, speak, and understand English, including interpreting documents such as safety rules and policy and procedure manuals.
  • Possess a current and valid NYS driver’s license with an acceptable driving record.
  • High School Diploma or GED.

Salary/Compensations

  • $20.17-$24.67 per hour
