About the Role
The Service Coordinator will provide general case management and referral services, linking ESSHI qualifying tenants to supportive services or medical services. This role involves assessing service needs, determining eligibility for public services, and coordinating service delivery with state and local agencies.
Responsibilities
- Provides linkage and referral to services for ESSHI qualifying tenants.
- Coordinates appropriate benefits, entitlements, and services.
- Evaluates health, psychological, and social needs and links to needed services.
- Provides formal case management with assessment and development of an individualized service plan.
- Maintains up-to-date knowledge of senior issues, entitlement programs, and community programs.
- Participates in case conferences and service plan meetings.
- Provides information and assistance to tenants and participates in tenant move-outs.
- Completes annual training in areas such as the aging process, elder care, disability services, and mental health issues.
- Maintains confidentiality and processes sensitive information according to agency policies.
- Provides representation for benefit eligibility interviews (e.g., Medicaid, Food Stamps, Medicare, Social Security).
- Responsible for oversight of students, volunteers, aides, or other assigned individuals.
- Provides eviction prevention activities.
- Conducts bi-annual surveys to assess resident needs and interests.
- Provides information on-site 12 times a year from community agencies.
- Advocates on behalf of tenants and families.
- Educates tenants, families, and staff on available community resources.
- Acts as a liaison between community agencies, tenants, and families.
- Provides on-going monitoring of service delivery.
- Completes all required documentation.
- Provides crisis management as needed.
- Compiles and maintains resource materials.
- Complies with all agency policies and procedures.
- Performs other duties as assigned.
Requirements
- Bachelor’s degree with a preference for gerontology, psychology, or counseling background.
- Service Coordinator/Case Manager experience with senior population preferred.
- Working knowledge of the aging process and awareness of community services and programs preferred.
- Valid Driver’s License that meets agency standards.
- Ability to travel to various locations in Erie, Niagara, Chautauqua, and Monroe County.
- Ability to organize, plan, and manage caseloads.
- Must possess sound judgment, flexibility, dependability, and good writing skills.
- Ability to handle multiple tasks simultaneously.
