About the Role
The Care Navigator is a core member of an Integrated Care team, responsible for coordinating and supporting mental health care within the clinic and facilitating referrals to external services. This role may involve providing evidence-based treatments or collaborating with other mental health providers.
Responsibilities
- Support and coordinate behavioral health care with the patient, patient’s Primary Care Provider (PCP), BHC, psychiatric provider and other Integrated Care team members.
- Screen and assess patients for common mental health and substance abuse disorders using identified screening tools.
- Provide patient education about common mental health and substance abuse disorders and available treatment options.
- Monitor patients for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management prescribed by PCPs and/or psychiatric provider.
- Provide brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, or other appropriate treatments.
- Provide or facilitate referrals to mental health clinic/substance use disorder treatment programs as clinically indicated.
- Participate in regularly scheduled Integrated Care team meetings and communicate treatment recommendations.
- Facilitate patient engagement and follow-up in care.
- Maintain a registry of patients in the Integrated Care track, including engagement and measurement-based care outcomes.
- Facilitate the Integrated Care team’s review of patients not responding optimally to treatment.
- Handle confidential information with discernment and sensitivity.
Requirements
- Minimum of 2 years of experience in Human Services related field, medical related field, customer service, client/patient interfacing experience required OR Associate’s degree required, bachelor’s degree preferred.
