About the Role
Implements and documents treatment programs. Provides superior customer service by modeling the Brand Promise and Core Values.
Responsibilities
- Completes and documents assessments, establishing objective, achievable, and measurable long and short-term goals within established standards of practice.
- Implements individualized treatment programs, issues and instructs in the use of adaptive equipment, and documents patient response.
- Communicates patient care plans, concerns, and status changes directly to the therapist.
- Participates in discharge planning, provides teaching to facilitate discharge, and suggests appropriate post-discharge equipment.
- Coordinates patient schedules, adjusts schedules to meet changing needs, and documents patient charges and attendance.
- Maintains professional growth through continuing education, in-services, professional organizations, and literature review.
Requirements
- Associate's Degree or Certificate in Occupational Therapy or Physical Therapy from an accredited school.
- Successful completion of all fieldwork requirements and eligibility to sit for PTA or OTA exam (Rehab Only).
- Minimum of 2 years post-graduate experience as a COTA in an acute care setting and/or an acute rehabilitation facility (HHCS Only).
- Licensed and/or registered by the State of New York.
- Reads, writes, and speaks English.
- Excellent interpersonal skills.
