About the Role
The WellBe care model is a Physician Led Advanced Geriatric Care Program focused on the quality of care of the frail, poly-chronic, and elderly Medicare Advantage patients. This population is often underserved and faces challenges accessing healthcare. WellBe brings care directly to the patient, offering a full continuum of care in various settings, including home, hospital, and skilled nursing facilities. Our teams partner with primary care physicians to provide high-level geriatric medical care and social support.
Responsibilities
- Answer incoming and outgoing calls.
- Coordinate care with the medical team.
- Schedule patient appointments, specialist appointments, and diagnostic tests.
- Act as a community resource.
- Engage patients who are not currently engaged.
- Contribute complete and accurate data to patient records.
- Update and maintain all digital client records.
- Assist in transitioning hard copy records to digital format.
- Conduct patient scheduling, registration, and eligibility verification.
- Manage patient medical records as appropriate.
- Answer patient non-clinical questions and explain processes.
- Schedule/coordinate WellBe provider visits, tests, and social resources.
- Ensure all appropriate patient information is in the EMR.
- Provide positive customer service and treat all patients and staff with respect.
- Prioritize urgent patient needs, scheduling needs for Complete Care Assessment, and efforts to engage the unengaged.
- Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
- Support APC in ongoing panel management and quality of care efforts (e.g., HEDIS).
- Complete next day visit chart prep.
- Utilize reporting to help providers track, meet and exceed team goals.
- Perform other tasks needed to accomplish team objectives/goals.
Requirements
- High school graduate or GED.
- 2+ years of experience working for a healthcare practice or hospital.
- EMR documentation experience preferred.
- High level of professionalism.
- Strong critical thinking skills.
- Strong customer centric focus/service skills.
- Strong computer skills, including Word, Excel, and PowerPoint.
- Strong verbal, written, presentation, and interpersonal communication skills.
- Ability to lift up to 20lbs.
- Ability to stand for extended periods.
- Ability to drive to patient locations (e.g., home, hospital, SNF).
- Fine motor skills/Visual acuity.
- Must be legally authorized to work in the United States without the need for employer sponsorship.
- Must successfully complete a pre-employment drug screening.
- Must successfully complete a background check.

- WellBe’s Care Model is to provide our members the entire continuum of care from chronic to urgent care in the home, hospital, skilled nursing facility, assisted living, palliative care, and to end of life care.
- WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.
