About the Role

The Family Support/Early Childhood Specialist is responsible for the ongoing management of an average of 15 children and families, including goal planning, development, and case record keeping. This role involves planning and advocating for services to assist families, administering developmental screenings, and other designated responsibilities.

Responsibilities

• Manage a caseload of children and their families to plan long and short-term goals.
• Conduct weekly home visits.
• Develop and maintain weekly case records of interactions and progress towards objectives.
• Administer timely developmental screenings for program participants.
• Encourage parent participation in program and agency activities.
• Conduct Community Outreach.
• Function as a mandated Child Abuse and Neglect Reporter.
• Prepare all release and consent forms.
• Submit all necessary statistical data and forms required by funding sources.
• Administer participant satisfaction surveys.
• Write brief weekly reports and quarterly reports for the Program Supervisor.
• Ensure timely completion and submission of all required documentation.
• Collaborate with agency partners and community service providers.
• Participate in staff development opportunities and in-service training.
• Attend Intake Meetings and Service Plan Reviews for the Foster Care Prevention Program.
• Conduct bi-weekly home visits for Prevention families.
• Ensure all Prevention families receive ages and stages screening.
• Participate in ongoing case management, planning, and advocacy services for Prevention families.
• Act as an advocate for clients in obtaining and/or maintaining supportive services during case reviews.
• Keep staff informed of family program status.
• Complete and submit progress notes, FASP case updates, and discharge summaries for Prevention families.
• Assume other job-related duties as assigned.

Requirements

• BS in Education and/or Human Services preferred or an AA Degree with related experience working with young children and their families.
• Knowledge and experience working in the field of Developmental Disabilities is preferred.
• Considerable work experience and professional training may be substituted for the educational requirement.
• Must use own vehicle.
• Must possess a Valid New York State Driver's License and be in good standing.
