About the Role

Responsibilities
- Manage a patient panel with diverse needs, conducting acute, chronic, and well visits with support from an onsite clinical Preceptor.
- Provide patient care in-office, testing centers, and through occasional telehealth visits.
- Engage in continuous learning through weekly Didactics, Clinical Rounds, and other learning modalities.
- Collaborate with in-office and virtual clinical teams through daily huddles.
- Utilize specific clinical training and perform in-office procedures.
Requirements
- Must have completed an accredited NP or PA program with national certification, graduating between November 2025 and May 2026.
- Must hold an active license in CA or have submitted an application for licensure by June 1, 2026.
- Must be Board Certified as a FNP, ANP, or PA.
- Demonstrate a passion for culturally competent primary care.
- Possess the ability to communicate with and provide care to individuals of all backgrounds.
- Effectively use technology to deliver high-quality care.
- Communicate effectively with patients and team members.
- Exhibit clinical proficiency in evidence-based primary care.
- Show a desire to be an integral part of a team dedicated to changing healthcare delivery.
- Be open to feedback and reflection for productive self-assessment.
- Confidently navigate uncertain situations with patients and colleagues.
- Adapt personal and interpersonal behavior to meet patient needs.
