About the Role
Provides psychosocial services to patients, working with the health care team to promote positive adjustment, rehabilitation, and improved quality of life. Supports the facility's commitment to quality indicators and actively participates in process improvement activities. This is an entry-level MSW role.
Responsibilities
- Assesses patients' psychosocial status, strengths, and needs that may affect rehabilitation and treatment outcomes.
- Participates in care planning with patients and the healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life.
- Utilizes patient education programs, social work theory, and quality of life measurements for assessment and care planning.
- Monitors and intervenes to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Provides supportive counseling services within the scope of clinical training and state license.
- Offers educational and goal-directed counseling to patients seeking transplant.
- Provides information and assists with referrals to community resources to facilitate optimal treatment outcomes.
- Maintains current knowledge of local vocational/educational rehabilitation programs and assists patients with referrals.
- Assesses patient awareness of advance directives and facilitates discussions if necessary.
- Participates in discussions regarding patient DNR status to ensure informed decisions.
- Adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation.
- Documents MSW interactions and interventions provided to patients and/or families.
- Provides psychosocial support and Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns.
- Participates in monthly Quality review meetings and reports on quality indicators.
- Assesses patient knowledge of kidney disease for barriers affecting treatment adherence and provides tailored education.
- Provides information about all treatment modalities with the interdisciplinary team.
- Facilitates the transplant referral process and collaborates on waitlist management.
- Provides ongoing education to patients/families regarding psychosocial issues related to ESRD and available support services.
- Reviews patient rights and responsibilities, grievance information, and facility policies with patients/representatives.
- Collaborates with the team on appropriate QAI activities.
- Reviews Patient Rights and Responsibilities, Grievance Procedure, and other relevant documents upon admission.
- Supports the clinic in patient scheduling to maximize adherence and adjustment.
- Interviews patients to identify root causes for discharge requests and shares concerns with leadership.
- Collaborates with Insurance Coordinators to address insurance-related issues.
- Provides information and education to patients about dialysis payment options.
- Collaborates with Insurance Coordinators regarding changes in patient state that impact insurance.
- Refers patients to the patient billing solutions (PBS) department for bill-related questions.
- Assists with the interview process and hiring decisions for new personnel if requested.
- Works with administrative staff to maintain updated patient resource lists.
- Provides training to staff on psychosocial topics as needed.
- Contributes to and participates in weekly team huddles, discussing urgent patient issues.
- Adheres to work defined caseload guidelines based on state regulatory requirements.
- Performs other related duties as assigned.
Requirements
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
