About the Role
The University of Rochester is seeking a social worker to provide clinical social work services, including transfer management and discharge planning, to individuals and families in the Emergency Department. The role also involves consulting with inpatient staff on psychosocial issues and community resources during specific hours.
Responsibilities
- Provides social work clinical services, including transfer management and discharge planning, to individuals and families.
- Consults with inpatient staff regarding psychosocial issues and community resources during specific hours.
- Evaluates the psycho-social status of patients and family to determine specific needs for service.
- Provides assistance through counseling and direct services to patients and their families.
- Discusses treatment and disposition plans with patients.
- Helps patients and families cope with home and community problems that hinder healthcare.
- Evaluates and makes recommendations regarding hospital release, home care, financial assistance, use of clinics, custodial institutions, and rehabilitation services.
- Makes home visits when necessary.
- Refers patients and families to community resources to maintain continuity of service.
- Provides social service consultation to other hospital staff.
- Attends rounds and staff meetings to coordinate hospital and clinic staff efforts for patient welfare.
- Helps develop rehabilitation plans by understanding patients and mobilizing resources.
- Determines the need for social work services and seeks clarification when necessary.
- Establishes and maintains cooperative working relationships with community agencies.
- Orients medical students and other hospital staff to the psycho-social aspects of illness and services.
- Provides information on psycho-social needs and problems of individuals with specific diseases or medical problems.
- Teaches casework, group work, and community organization to residents, medical students, nurses, and trainees.
- Attends and participates in departmental, hospital, professional, and special meetings.
- Meets with members of community agencies to share information, procedures, and techniques.
- Maintains knowledge of social work practices and community agency policies.
- May supervise lower-level support staff and students and evaluates performance.
- Assists in the orientation and training of new staff members.
- Arranges and carries out case conferences.
- Makes recommendations on procedures and policies of the Division of Social Service.
- Completes recording on all cases according to established policies.
- Conducts correspondence with other institutions and community agencies.
- Performs other duties as assigned.
Requirements
- Master's degree from an accredited school of social work required or equivalent combination of education and experience.
- LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required.
- If duties will be performed under OMH, a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required.
