About the Role
This field-based, clinical support role provides direct therapeutic interventions, behavioral management, and care coordination for children with complex needs in the foster care system. The specialist acts as a liaison between the treatment team, foster parents, and community resources to foster stability, skill development, and family reunification.
Responsibilities
- Provide direct services to children assessed as needing therapeutic services.
- Attend FTC and other related case conferences for children assessed as needing clinical support.
- Assist foster parents and children in carrying out the treatment plan.
- Conduct home visits and collateral contacts to meet ACS and OCFS requirements.
- Provide in-home and in-community behavior management, therapeutic, and developmental support to children with complex needs.
- Support specific behavioral goals as developed by the treatment team.
- Create, develop, and implement daily experiences for children that promote self-esteem, socialization skills, and prepare children for family unification.
- Perform child/youth care functions relevant to daily home care, ADL skills, scheduling, respite planning, and crisis intervention.
- Document progress notes in CONNECTIONS.
- Perform other related duties as assigned by supervisor.
Requirements
- BA/BS degree with 2 years of experience working with children and families.
- Excellent verbal and written communication skills.
- Computer proficiency.
- Knowledge of CONNECTIONS.
- Bilingual (English/Spanish) is a plus.
- Authorized to work in the U.S.
- Ability to work in-person in New York City (NY) for at least 3 days per week.
- Ability to travel to other Rising Ground sites (if required).
