About the Role

The Care Coordinator supports logistical patient care activities for Mount Sinai at Home, performing remote duties to assist with home-based care orders, coordinating with vendors, and ensuring patients receive necessary services. This role involves administrative tasks, clear communication, and collaboration with clinical teams, patients, and vendors, while sharing accountability for clinical quality and safety outcomes.

Responsibilities

Works as a member of an interdisciplinary care team assessing and addressing complex patient and family care needs.
Supports admission and ongoing home-based census operations and follows up on orders to coordinate care.
Provides administrative support for tracking ongoing care activities, including vendor liaison, equipment pickup coordination, appointment scheduling, and discharge order confirmation.
Communicates the plan of care and assessments to front-line teams.
Participates in daily/weekly huddles/IDT meetings with care teams.
Meets with leadership on a regular cadence (minimum monthly) to discuss quality metrics, provide feedback, and propose solutions for program workflow expansion.
Manages and maintains the operational database, including relevant care coordination and care delivery metrics.
Serves as a liaison between physicians, clinical staff, and vendors.
Sets priorities, problem-solves, and uses proper judgment in difficult situations.
Monitors Epic orders.
Promptly answers calls from clinical teams.
Ensures patient privacy by adhering to MSHS information security policies, procedures, and HIPAA guidelines.
Maintains accurate records of interventions and safety events.
Submits utilization and assignment reports every shift to supervisors and site leadership.
Maintains attendance and punctuality records in accordance with department procedures and/or policy.
Performs other assigned duties and responsibilities.

Requirements

Minimum one year of healthcare experience.
Knowledge or experience with care coordination, patients, families, and clinical staff.
Ability to identify data elements for quality improvement/quality assurance.
Experience drafting organizational protocols and workflows.
Experience working with EMR.
Strong telephonic and coordination organization skills.
Proven ability to identify strategies for measuring and communicating program successes and lessons learned.
Three years of healthcare experience preferred.
