About the Role
Wellness Coaches are needed to assist consumers with accessing community services, developing a referral network, and transforming lives. This role offers career progression opportunities within the not-for-profit social services sector.
Responsibilities
- Engage people with mental illness to teach skills and link them to services supporting recovery, independence, and self-determination.
- Conduct Group Meetings on Wellness Management.
- Complete service plan reviews for all residents.
- Assess, correct, and report on apartment and personal living space inspection conditions.
- Provide Case Management for residents, engaging them in setting rehabilitation and housing support goals, and collaborating on Housing Support Plans.
Requirements
- Bachelor’s Degree in human services or related field required.
- A minimum of two years’ experience working with people who have a mental illness.
- Computer literate in word processing and excellent writing capabilities to develop treatment plans.
