About the Role

Evaluate and direct nursing care of residents on a particular nursing care unit, integrating the philosophy and objectives of the Nursing Home and Nursing Department into the management of nursing care.

Responsibilities

  • Initiate plan of care by assigning resident care based on resident need and staff abilities.
  • Observe residents' physiological and psychological behavior, notify the NCC of concerns, and document observations.
  • Provide continuity of care for all residents on the assigned unit and cooperate in providing continuity of care across all shifts.
  • Provide updates regarding the plan of care to residents, families, nursing staff, and other personnel.
  • Participate in resident teaching programs.
  • Contribute to resident care plans, defining needs and concerns.
  • Discuss administrative and nursing care problems with Nurse Care Coordinator and/or DON.
  • Review physicians' orders daily and ensure they are programmed accordingly.
  • Complete Quarterly Nursing Summaries for each resident on the assigned unit.
  • Participate in Quality Assurance activities as directed.
  • Stay informed of new medical and nursing measures related to resident needs.
  • Maintain an environment conducive to quality care.
  • Make rounds to observe care effectiveness and consult with Nurse Care Coordinator for necessary changes.
  • Provide skilled nursing care to residents as ordered.
  • Demonstrate new nursing techniques per facility policies.
  • Maintain a system for communication of nursing care information for each resident.
  • Make rounds with physicians as needed.
  • Participate in nursing and resident care conferences.
  • Maintain nursing documentation in accordance with facility policies and procedures.
  • Use the nursing care plan as a guide for reports, rounds, conferences, and assignments.
  • Share information with other para-medical groups and professional disciplines.
  • Review critical incidents and discuss with Nurse Care Coordinator/DON.
  • Assure responsibility for teaching related to planning, implementing, and evaluating nursing care for each resident.
  • Observe nursing care requirements relative to resident numbers, condition severity, treatment plans, and nursing activities, reporting concerns to the Nurse Care Coordinator.
  • Assign personnel in accordance with resident needs and technical competency.
  • Identify nursing care problems and personnel assignment issues.
  • Participate in In-service and orientation programs.
  • Supervise and counsel direct care personnel assigned to the unit.
  • Create and maintain a working climate conducive to quality care.
  • Assure direct care staff work assignments are completed daily.
  • Respect individual abilities, knowledge, and skills.
  • Plan opportunities for growth and development.
  • Guide working relationships with residents, families, visitors, nursing personnel, medical staff, other departments, and community agencies.
  • Support personnel policies.
  • Recommend changes to Nursing Supervisors/DON.
  • Keep personnel informed of changes.
  • Interpret new, current, and standing policies for staff members.
  • Review all incident reports and discuss prevention with staff.
  • Maintain an effective communication system on the unit.
  • Report personnel problems to the Nurse Care Coordinator.
  • Follow Standards of Practice and facility policies for medication and treatment administration.
  • Manage the narcotics cabinet, including record maintenance, receipt, and use when assigned as med nurse.
  • Support new programs and policies.
  • Suggest policy changes to Nurse Care Coordinator/DON.
  • Notify appropriate disciplines for maintenance or upkeep of equipment and supplies on the unit.
  • Check emergency equipment weekly (O2, oral suctioning, ambu bags, CPR board) and report concerns to Maintenance and Nursing Administration.
  • Ensure the nursing unit is clean, neat, and functional (dirty utility room, clean utility room & kitchen).
  • Ensure each resident is treated with dignity and respect, reporting concerns to Administration.
  • Follow established infection control procedures.
  • Participate in in-service and continuing education programs to stay current in nursing.
  • Keep LPN license current and provide a copy for personnel file.
  • Adhere to facility Code of Conduct, including professionalism and service excellence.
  • Assist in other duties as deemed necessary by Supervisor, including providing care and assisting residents as needed.
  • Work on other shifts or units as needed to maintain acceptable staffing levels.
  • Attend all required in-services and follow all department and facility policies and procedures, including PPE use.
  • Maintain confidentiality of resident care and employee information, following HIPAA and HIV regulations.

Requirements

  • Graduate from an accredited school of nursing.
  • Current licensure by the State of New York as a Licensed Practical Nurse.
  • Minimum 1 year experience as an LPN demonstrating potential supervisory competence (preferred).
  • BLS Certification within 6 months of hire.
