About the Role
The Health Home Care Manager collaborates with youth, families, providers, and community resources to stabilize and enhance the health and well-being of enrolled children through care plan development, monitoring, and evaluation.
Responsibilities
- Verify eligibility of youth referred for Health Home Care Coordination services.
- Conduct initial and ongoing assessments of youth and family strengths and needs.
- Develop and promote adherence to interdisciplinary care plans, including crisis response.
- Monitor progress and evaluate the effectiveness of care plans, making adjustments as needed.
- Serve as the primary contact, advocate, and resource for youth, families, and the care team.
- Build relationships with service providers and agencies through timely communication.
- Provide education and facilitate linkages to community resources for youth and families.
- Complete all required training within prescribed timeframes.
- Ensure compliance with agency and program policies and procedures.
- Participate in performance improvement activities.
- Participate in on-call rotation as necessary.
- Perform other associated tasks as assigned by Supervisor.
Requirements
- Master's degree in social work, Psychology, or related field with at least one year of Human Services experience.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) with at least one year of Human Services experience.
- Bachelor's Degree in Social Work, Psychology, Sociology, or related field with at least two years of Human Services experience.
