About the Role
The Medical Assistant implements medical/health-related tasks under the direction of the RN Clinical Coordinator and Program Manager. The Habilitation Aide supports individuals with personal hygiene, dining, ambulation, transportation, behavioral support, and overall health and well-being, ensuring services align with individual needs and choices.
Responsibilities
- Implement medical/health-related tasks as directed and overseen by the RN Clinical Coordinator of Day Services and Program Manager.
- Implement each individual’s Individualized Program Plan.
- Assist with personal hygiene, dining, ambulating, transportation, and behavioral support.
- Ensure the overall health and well-being of the individual.
- Meet regularly with supervisor and confer with other members of the individual’s Service Planning Team.
Requirements
- High school diploma or GED preferred.
- Verbal and written communication skills are necessary.
- One year of experience working with adults with developmental disabilities preferred.
- Experience with medical appointments and medication ordering and administration required.
- Must maintain a valid driver’s license and driving record that meets Pathways insurability guidelines.
- Must meet requirements related to Criminal History Record checks, including fingerprinting if applicable.
