About the Role
A Psychosocial Rehabilitation Specialist serves in a paraprofessional capacity within the crisis intervention unit, assisting clients with various aspects of their care and recovery.
Responsibilities
- Assists clients with paperwork for admissions, discharges, and post-discharge phases.
- Transports patients/clients to and from psychiatric and crisis stabilization placements.
- Provides backup support for crisis worker phone contacts.
- Assists in coordinating care for clients in crisis stabilization sites.
- Performs in-person medication deliveries, welfare checks, and phone prompts.
- Helps clients develop and implement personal wellness goals.
- Provides rehabilitation support, recovery-oriented activities, interventions, and skill development.
- Assists individuals in identifying meaningful life role goals and objectives through a person-centered process.
- Trains clients in coping skills to manage symptoms, stress, and reduce vulnerability.
- Assists in identifying trauma triggers and developing healthy coping alternatives.
- Encourages personal autonomy through stress management and symptom recognition.
- Supports the development of ways to manage free time.
- Assists in navigating transportation.
- Guides clients in understanding and managing chronic medical conditions.
- Engages clients in developing social skills and wellness activities.
- Teaches and strengthens daily living skills.
- Encourages the development of financial management strategies.
- Assists clients in pursuing, securing, and retaining employment.
- Assesses client progress toward recovery and functional skill development.
- Maintains required documentation in a timely manner.
- Communicates and reports to Supervisor.
- Actively participates in assigned trainings.
- Manages a caseload of clients to establish and implement individual health and wellness plans.
- Assists in establishing a wellness support system outside of the PRS.
- Consults with collaterals (with client permission) to improve client outcomes.
- Provides psychoeducation to support community tenure and avoid restrictive placements.
- Enhances personal relationships and establishes support networks.
- Increases community awareness and develops coping strategies for effective functioning.
- Teaches and strengthens daily living skills for self-management.
- Supports personal autonomy and skill development.
- Enriches personal skills and develops health skills.
- Identifies personal interests and hobbies.
Requirements
