About the Role
Provides HIV counseling, testing, and retention services to patients, ensuring linkage to care and ongoing support within the HIV Medicine division.
Responsibilities
- Provides HIV counseling and testing services onsite and in the community.
- Engages, re-engages, and links new and existing patients to care.
- Conducts HIV counseling and testing at community events and for walk-in clients.
- Recognizes signs of HIV sero-conversion and notifies medical providers.
- Serves as a liaison to community agencies for counseling and testing.
- Maintains patient charts, documents, and educational materials.
- Facilitates provider completion of HIV reporting and Partner Notification forms.
- Follows guidelines for clients with newly positive HIV tests, including assuring confirmatory tests and arranging appointments.
- Links patients into HIV care within 30 days of confirmatory test.
- Coordinates with community and internal teams to facilitate access to HIV care.
- Collaborates with multidisciplinary teams to link newly positive HIV patients to care.
- Tracks patient information to monitor linkage and retention in care.
- Follows NYS HIV testing guidelines and AMC policy for pre- and post-testing.
- Conveys information regarding benefits of HIV testing, HIV/AIDS relationship, transmission, testing options, procedures, results, and the window period.
- Provides risk/harm reduction education and counseling.
- Adapts counseling techniques to patient's developmental level and circumstances.
- Responds to patient questions and concerns.
- Assesses patient's ability to cope with results and availability of support resources.
- Assesses patient's capacity to consent to HIV testing and obtains consent.
- Conveys HIV test results and addresses immediate concerns for negative test results.
- Discusses the possibility of HIV exposure during the "window period" and advises on re-testing for negative results.
- Reinforces risk/harm reduction strategies for negative results.
- Discusses medical care and follow-up services for positive test results.
- Provides education on behavior change to prevent transmission for positive results.
- Encourages and offers assistance with partner/contact notification for positive results.
- Provides referrals for partner and child HIV testing as needed.
- Provides referrals to medical and support services within AMC and the community.
- Utilizes knowledge of AMC and community resources to advocate for patient needs.
- Completes Pre-Intake Screening for new patients to prepare them for appointments and identify barriers.
- Provides reminder calls to patients with new HIV appointments.
- Assists with new patient intakes.
- Contacts patients post-visit to assess understanding of medical plan and identify barriers.
- Conducts quarterly follow-up with patients to assess needs, barriers, and understanding.
- Establishes caseloads for patients at risk for retention issues.
- Reviews medical records and collaborates with care teams to reach patients.
- Attempts to contact patients regularly to address barriers and schedule appointments.
- Notifies the team of patient retention status.
- Enlists assistance from community agencies when authorized.
- Contacts patients prior to appointments to remind and re-assess for conflicts/barriers.
- Meets patients at medical visits when possible.
- Checks in with patients following appointments or no-shows.
- Provides reminder calls and assesses barriers for upcoming medical visits.
- Monitors patients until successfully retained in care or all contact avenues are exhausted.
- Completes Ryan White Eligibility Forms.
- Enters counseling/testing, retention, care coordination, and other patient services into the AIRS database.
- Enters intake information into the AIRS database.
- Enrolls patients into specified AIRS programs.
- Closes patients from AIRS programs.
- Maintains spreadsheets as assigned.
- Documents all client/patient encounters and care coordination activities in paper charts or electronic health records.
- Uses DIP format for documentation.
- Ensures notes are objective and concise.
- Ensures impressions/opinions are supported by objective information.
- Ensures each note supports corresponding entries in AIRS.
- Utilizes EHR tasking feature to communicate with the care team.
- Copies tasks to notes or separate documents for care team communications.
- Performs additional duties as assigned.
Requirements
- Bachelor’s degree in social work, psychology or sociology.
- Prior experience working in a healthcare setting preferred.
- Ability to read and understand medical information.
- Knowledge of medical terminology is helpful.
- Fluency in written and oral English.
- Ability to calculate figures and amounts such as discounts, interest, proportions, and percentages.
- Good computer and typing skills.
- Ability to define problems, collect data, establish facts and draw valid conclusions.
- Regularly required to sit.
- Frequently required to walk.
- Occasionally required to stand.
- Occasionally lift and/or move up to 10 pounds.
- Specific vision abilities required include close vision and the ability to adjust focus.
- Able to handle moderate levels of stress.
- Occasionally exposed to infectious diseases.
- Access to information is based on a “need to know” and is the minimum necessary to properly perform assigned duties.
- Use or disclosure shall not exceed the minimum amount of information needed to accomplish an intended purpose.
- Reasonable efforts shall be made to ensure that information is adequately protected from unauthorized access and modification.
