About the Role
The Case Associate is an intermediate-level direct care staff member supporting adults with serious and persistent mental illness in Jewish Board supported housing programs. The role focuses on helping residents live independently in the community by providing services within program regulations and agency mission, assessing and supporting independent living skills, and advocating for residents.
Responsibilities
- Carry a caseload of residents and provide services within scope of program regulations and agency mission.
- Evaluate and assess independent living skills, supporting residents in meeting individual goals.
- Advocate on behalf of residents with other service providers.
- Link residents to community-based treatment and collaborate with other service providers.
- Practice person-centered and collaborative service planning with residents.
- Ensure service plans are completed and signed by residents according to regulatory guidelines.
- Interpret and explain agency policies and procedures to residents.
- Support residents toward medication management independence using person-centered interventions.
- Conduct monthly unit/apartment inspections.
- Provide resident escorts to appointments as needed.
- Assist residents with moving, carrying groceries, laundry, and other household items.
- Participate in regular staff meetings and trainings, potentially off-site.
- Participate in planning and execution of special and holiday events.
- Complete regular and timely documentation of all resident interactions, including progress and informational notes.
- Ensure progress notes meet monthly billing documentation requirements.
- File incident reports as appropriate.
- Facilitate monthly roommate meetings when appropriate.
Requirements
- Understanding of how mental illness, substance use disorders, or trauma can affect individuals in the community.
- Experience working with individuals living with serious mental illness, substance use disorders, or trauma required.
- Knowledge of local and state health and human service systems and their navigation.
- Understanding of the stabilization and recovery processes.
- Commitment to working with individuals with serious mental illness, substance use disorders, a history of trauma, and/or homelessness.
- Compassion and respect for vulnerable individuals.
- Openness to working through an anti-racist lens and continuous learning.
- Ability to remain calm in stressful situations.
- Patience and empathy for individuals on their recovery journey.
- Ability to travel to visit residents in apartments or escort residents in the community.
- Valid driver’s license and willingness to drive agency van preferred.
- Ability to lift 25 pounds to assist residents.
- Ability to manage stairs in walk-up buildings.
