About the Role

The LPN will utilize clinical experience to plan and deliver safe, therapeutic, and age-appropriate patient care. This role involves collaborating within a multidisciplinary health team to promote wellness and optimize patient outcomes, while adhering to all hospital policies, federal, state, and local regulations, including HIPAA guidelines.

Responsibilities

Perform other duties as assigned.
Complete documentation reflecting multidisciplinary coordination.
Document teaching/learning needs of patients and families.
Evaluate and document patient's response to nursing and multidisciplinary team interventions.
Evaluate patient's clinical status and attempt resolution of issues at point of care before escalating.
Take action to optimize patient outcomes.
Participate in the identification, investigation, and resolution of risk management, quality care, and ethical issues.
Address patient issues following established communication hierarchy.
Evaluate patient progress toward health goals and modify plan of care accordingly.
Interact and collaborate with other members of the health team to identify patient needs.
Initiate appropriate interventions to amend identified variances from the plan of care.
Document patient and family response to interventions and collaborate with the RN to modify the plan of care.
Provide direct patient care utilizing hospital policies, patient care guidelines, and physician's orders.
Incorporate standards of practice, clinical pathways, age-specific guidelines, and patient care guidelines into the plan of care.
Recognize and respond to issues that may cause problems and refer issues to the RN.
Utilize the escalation policy.
Recognize the family as defined by the patient and incorporate principles of family-centered care.
Identify patient satisfaction/dissatisfaction with the plan of care and escalate issues as required.
Verify and document patient advance care directives.
Maintain open communication through intra and inter-shift reporting.
Utilize appropriate communication in all interactions and demonstrate proper telephone etiquette.
Participate in maintaining a clean, safe, therapeutic environment, including security of patient belongings.
Respond to emergency situations as appropriate.
Ensure appropriate emergency equipment is present and operational.
Participate in implementing the HEICS plan as required.
Obtain prior authorization for medication from the patient's insurance company as needed.
Obtain referrals for patient tests ordered by MD as per patient's insurance requirements.
Maintain exam room for necessary supplies and cleanliness.
Process medication orders and refills through the Workflow system.
Verify dosage and medication using various tools.
Enter comments and initials in Workflow when receiving verbal approval from a physician to refill a prescription.
Implement conflict resolution skills and address issues with the plan of care if patient expresses dissatisfaction.
Assess and document patient/family advance care directives and Rights and Responsibilities if indicated.
Establish collaborative and collegial relationships with health care team members.

Requirements

High School or Equivalent Technical Diploma
Graduate of accredited school of licensed practical nursing
0 - 1 year nursing experience working in a hospital, ambulatory, private practice or home health care setting preferred.
Qualified candidates must be able to effectively communicate with all levels of the organization.
