About the Role
This role is responsible for ensuring regulatory compliance and quality improvement within Parker's Community Health Services Program, which includes a Certified Home Health Agency and a Comprehensive Community Hospice program. The position involves clinical and administrative oversight, educational support, and driving performance improvement to achieve optimal clinical outcomes.
Responsibilities
- Ensure regulatory compliance of Parker's Community Health Services Program.
- Oversee Quality Assurance and Performance Improvement measures.
- Provide clinical and administrative oversight of the program's quality program.
- Offer educational support to staff.
- Identify opportunities for performance improvement.
- Lead the program to improved clinical outcomes.
- Maintain high quality of care standards.
- Conduct home visits as needed.
Requirements
- Strong understanding of current CHHA and/or Hospice regulations.
- Knowledge of quality improvement measures.
- Clinical education skills.
- Prior staff experience working in a Certified Home Health Agency (CHHA) or Hospice Program.
- Prior Supervisory, Quality Assurance or Education experience preferred.
- Strong computer skills including EMR, report generation and Excel.
- Reliable access to transportation to conduct home visits as needed.
