About the Role

The Social Worker will complete full psychosocial assessments and collaborate with the CTMS interdisciplinary team to support trans patients through their medical transitions. This role is pivotal in facilitating appropriate medical interventions, confirming mental health evaluations and necessary documentation, and ensuring post-surgical resources are in place. The position also involves developing and maintaining support groups, providing short-term therapy, and participating in team and community meetings.

Responsibilities

• Complete full psychosocial assessments.
• Collaborate with the CTMS interdisciplinary team including nurse coordinator, eligibility assistant, mental health professionals, primary care providers, surgeons, medical specialists, and outside service providers.
• Support trans patients through their medical transitions.
• Facilitate appropriate medical interventions when required.
• Confirm patients have complete mental health evaluations, all necessary letters, and pre-surgical treatments.
• Determine that post-surgical resources for care in the community are in place.
• Collaborate with inpatient social workers, discharge planners, and outside agencies.
• Develop and maintain support groups for CTMS patients (pre- and post-surgical).
• Provide short-term therapy (pre- and post-op).
• Participate in CTMS team meetings, steering committee meetings, and community advisory board meetings.
• Create and organize patient lists with up-to-date surgical information.
• Co-lead case conference meetings to discuss high-risk patient issues.
• Mediate between in-patient and out-patient teams to facilitate smooth and comprehensive care throughout the surgical process.

Requirements

• Experience working with LGBTQIA+ individuals with specific familiarity with transgender individuals.
• Excellent engagement, organizational, collaborative, and clinical skills.
• Excellent administrative and time-management skills.
• A collaborative and flexible attitude to meet the changing needs of the interdisciplinary team.
• Bilingual (English/Spanish) preferred.
• NYS LMSW or LCSW
