About the Role

The Clinical Nurse for Physician Access Services is a Registered Professional Nurse responsible for completing intakes and triages to clinical programs or services using a decision algorithm for designated patient populations. This position coordinates and facilitates patient referrals into the Mount Sinai Health System, working at Mount Sinai's corporate office near Grand Central. The nurse collaborates with a dynamic group of nurses and coordinators to help patients access healthcare across the entire health system.

Responsibilities

Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
Serves as the point of contact for physician referrals and obtains necessary intake information to initiate the patients referral into Mount Sinai Health System.
Conducts site visits to maintain key partnerships with existing and potential physician practices, hospital affiliates, or any other identified referral base.
Advocates for patients, families, and referring physicians to promote overall satisfaction with accessing and utilizing Mount Sinai Health System.
Communicates information between patient, referrer, family, and insurance company as indicated.
Assists the Patient Transfer Service with coordinating hospital transfers into the Mount Sinai Health System using an established decision algorithm.
Promptly answers and screens inquiries with strict adherence to confidentiality agreements, policies and procedures.
Performs insurance verification and obtains authorization as needed.
Integrates relevant assessment and intervention skills in the delivery of patient care coordination.
Initiates service recovery for patients and family members who have concerns and escalates to Supervisor, Manager/Clinical Manager/Director as needed.
Demonstrates caring, respect, compassion, empathy and active listening through dialogue and actions.
Approaches patients, families and other members of the health care team in a professional, respectful manner.
Responsible for the complete and accurate registration of all patients in an expeditious and positive manner.
Documents all patient encounters and enters all data into referral tracking system.
Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol.
Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives.
Performs nurse duties as assigned.
Escalates material or staffing resource deficits to Management team.
Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources.
Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care.
Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards.
Works a schedule that includes rotating weekends and holidays.
Performs other duties as assigned by Manager and Director.
Maintains clinical competence and highest standards of nursing practice.
Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team.
Projects a professional image to colleagues and communicates with styles and methods that demonstrate caring, respect, compassion and empathy.
Incorporates ethical principles into decision making for patient and family.
Encourages and demonstrates a spirit of scholarship, inquiry, life-long learning and innovation for self and others.
Acquires knowledge and skills relative to the role, patient population, clinical specialty and local and/or global health community needs.
Participates in shared decision making through specialty practice councils, nursing department committees and unit initiatives.
Serves as a professional role model and preceptor for new staff and students.
Advances as a clinical leader through acting in the charge role, as a patient care coordinator facilitator and resource person for the unit/clinical service area.
Articulates the values of research and evidence-based practice and its application to nursing practice and the environment of care.
Contributes to the profession of nursing through participation in professional organizations.

Requirements

1-2 years acute care medical/surgical experience.
Desire to work in a behind the scenes setting and use your clinical skills is essential.
