About the Role

The Social Work Services at The Mount Sinai Hospital provides comprehensive social work services and programming to meet the needs of diverse populations. Social Workers collaborate within interprofessional teams to serve patients and the community, focusing on physical, emotional, and social needs. This role specifically supports trans-identified patients interested in gender-affirming medical interventions, including surgeries, by working within a multidisciplinary team approach.

Responsibilities

Complete full psychosocial assessments and collaborate with the CTMS interdisciplinary team to support trans patients through medical transitions.
Facilitate appropriate medical interventions when required.
Provide surgical support, including confirmation of mental health evaluations, necessary letters, pre-surgical treatments, and determination of post-surgical community resources.
Develop and maintain support groups for CTMS patients (pre- and post-surgical).
Provide short-term therapy for pre- and post-operative patients.
Participate in CTMS team meetings, steering committee meetings, and community advisory board meetings.
Perform administrative duties such as creating and organizing patient lists with surgical information.
Co-lead case conference meetings to discuss high-risk patient issues.
Mediate between in-patient and out-patient teams to facilitate comprehensive care throughout the surgical process.

Requirements

Experience working with LGBTQIA+ individuals, with specific familiarity with transgender individuals.
Excellent engagement, organizational, collaborative, and clinical skills.
Excellent administrative and time-management skills.
A collaborative and flexible attitude to meet the changing needs of the interdisciplinary team.
Minimum one year in current position.

Skills

Bilingual (English/Spanish) preferred.
