About the Role
Support residents in developing the skills needed to live independently and thrive in their communities.
Responsibilities
- Develop and implement Individual Action Plans (IAPs)
- Provide approved Medicaid billable services, including Assertiveness and self-advocacy, Community integration, Daily living skills training, Health services and medication management, Parenting training, Rehabilitation counseling and skill development, Socialization and symptom management, and Substance use support
- Teach and support independent living skills, such as Nutrition, meal planning, grocery shopping, and food preparation/storage, Basic housekeeping, Personal hygiene, and Money management
- Encourage positive social interaction and participation in recreational and leisure activities
Requirements
- High School Diploma or GED
- Valid NYS driver's license
- First Aid/CPR certified (preferred)
