About the Role
Sun River Health is seeking a Hepatitis C Linkage to Care Specialist to provide education and outreach for patients who are HCV positive, working with the Director of Nutrition and Operations.
Responsibilities
- Facilitates enrollment in HCV Benefits and Entitlement programs
- Maintains data and case records as required and prepares necessary reports
- Educates community agencies and internal staff about the importance of Hepatitis C testing and treatment
- Educates providers and other staff about HCV testing, benefits of diagnosis and importance of linking to care
- Works cooperatively and collaboratively with the entire HCV case management, Medical and Behavioral Health Provider teams
- Provides outreach to testing centers, educates those newly diagnosed with Hepatitis C and refers to SRH, including tracking of all services
- Reduces barriers to ensure timely access to care and treatment
- Develops linkages and internally refers patients for additional medical services, including treatment
- Works with Community Based Organizations (CBOs) to set up HCV screening services, provide community education and outreach and acts as a liaison with CBOs to link HCV positive patients to care
- Participates in case conferences and other agency or community-based meetings or trainings
