About the Role
The Clinician - Licensed is responsible for maintaining a client caseload, identifying goals and objectives to improve emotional, social, and daily functioning with clinical supervisor support.
Responsibilities
- Complete a comprehensive biopsychosocial history assessment including patient problems, strengths, preferences, cultural, religious, and ethnic background.
- Assist patients in creating and maintaining person-centered treatment plans.
- Attend weekly staff meetings, treatment teams, and supervision.
- Complete mandatory documentation on EHR within established regulatory timelines.
- Empower patients and encourage choice in matters affecting them, demonstrating sensitivity to racial, cultural, ethnic heritage, and sexual orientation.
- Provide linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs.
- Provide patients and families with crisis management services by appointment or walk-in.
- Develop a comprehensive discharge plan in collaboration with patients, families, multidisciplinary staff, and referral agencies.
- Provide individual, group, and/or family counseling for mental health and/or substance use.
- Collaborate with other local agencies such as DSS, Probation, and Drug Court.
- Comply with OMH, OASAS, New York State and Federal regulations, agency policies, procedures, laws, and ethical guidelines.
- Maintain training, licenses, and certifications per regulatory and agency requirements.
Requirements
- Master’s degree in social work or mental health from an accredited school.
- Licensed eligible.
- If licensed, required to track and maintain status of professional licensure.
- Continuing professional education required.
- Experience working with mental health and substance use population is preferred.
