About the Role
Elderplan and HomeFirst are Medicare and Medicaid managed care health plans that provide high-quality healthcare designed to help people live independently on their own terms.
Responsibilities
- Provide case management to members, addressing medical, social, psychological, physical, and spiritual needs.
- Develop, implement, and monitor care plans in conjunction with the PCP, caregivers, and other team members to meet member needs.
Requirements
- Associate’s Degree in Nursing for experienced candidates
- BSN for new graduates
- NYS RN license required
- CCM preferred
- 1 year of RN experience in managed care, home care, acute care or nursing home preferred
Experience Level
- 1 year of RN experience in managed care, home care, acute care or nursing home preferred
Education Level
- Associate’s Degree in Nursing
- BSN
Salary/Compensations
- USD $90,000.00/Yr. - USD $110,000.00/Yr.
