About the Role
CHPP Ambulatory Care Lake Success is seeking an experienced Nurse Practitioner (NP) to join our Gastroenterology practice. This full-time, primarily outpatient position supports a high-volume Gastroenterology physician and expands access to high-quality specialty care. The NP will work closely with Dr. Christine Yu and the multidisciplinary practice team to provide comprehensive patient care, clinical triage, patient communication, and provider support, increasing patient access and continuity of care.
Responsibilities
- Provide comprehensive clinical care to Gastroenterology patients in the outpatient setting.
- Conduct patient assessments, follow-up visits, and appropriate management of Gastroenterology conditions.
- Perform clinical triage and determine appropriate follow-up, testing, treatment, or escalation of care.
- Communicate with patients regarding clinical concerns, test results, treatment plans, and follow-up needs.
- Provide clinical and administrative support to the collaborating Gastroenterologist.
- Coordinate care with physicians, nurses, medical assistants, and other members of the healthcare team.
- Promote continuity of care and timely access to Gastroenterology services.
- Support practice efficiency and patient throughput in a high-volume clinical environment.
- Participate in hospital-based care or rounding in the future, as practice needs evolve.
Requirements
- Current Nurse Practitioner licensure in New York State.
- Graduate degree from an accredited Nurse Practitioner program.
- National board certification as a Nurse Practitioner in the applicable specialty.
- Current BLS certification.
- Current ACLS certification.
- DEA registration/certification.
- Previous Gastroenterology experience is required.
- Strong clinical assessment, communication, and patient-management skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- New graduates will not be considered for this position.
