About the Role
The Oncology Nurse Coordinator facilitates the care of cancer patients undergoing active treatment at Stony Brook Medicine. This role establishes and coordinates a care map, acts as a liaison and support system, and serves as a patient resource during treatment.
Responsibilities
- Serves as a liaison for assigned physician practices.
- Formulates and updates the plan of care addressing educational, financial, and social work needs.
- Performs reassessments including psychological, physical, environmental, and functional needs, and makes referrals.
- Facilitates communication between attending staff, residents, fellows, nurses, and secretarial support staff.
- Follows patients through their surgical or medical oncology course of care.
- Provides patient and family education regarding their disease-specific plan of care.
- Assists with the development and/or update of policies and procedures related to coordination.
- Assists in the completion of cancer survivorship care plans in coordination with nurse navigators and the disease management team.
- Provides patient/family education and explanations about the course of treatment and care.
- Participates in and contributes expertise to activities and functions that strengthen the cancer program, including continuous program and quality improvement.
- Coordinates and schedules treatment and clinic visits, ensuring timely care.
- Facilitates the flow of clinic and treatments during assigned physician sessions.
- Orients new patients to the service.
- Performs ongoing assessments and communicates changes to physician and nursing staff.
- Receives and returns patient inquiries regarding symptom management and their plan of care.
- Performs follow-up telephone calls to patients after their first treatment.
- Creates and maintains a binder or electronic document on all active treatment patients.
- Communicates problems or barriers to direct supervisor for problem-solving.
- Ensures chemotherapy orders are available to infusion staff two days prior to the infusion visit.
- Actively seeks and participates in educational opportunities to achieve and maintain knowledge and skill related to job responsibilities.
Requirements
- NYS RN license.
- Three (3) years acute nursing experience, with a minimum of 2 years in oncology.
- Bachelor’s degree in Nursing or current enrollment in a BSN program (proof must be submitted).
- Verification of degree (e.g., diploma or official transcript) is required.
- Resume/CV should be included with the online application.
- Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services.
- Complete electronic reference check with a minimum of three (3) professional references.
- Successfully complete a 4 panel drug screen.
- Meet Regulatory Requirements for pre-employment screenings.
- Provide a copy of any required New York State license(s)/certificate(s).
