About the Role
The RN Case Manager will administer skilled nursing care to patients requiring professional nursing service, teach and supervise the family and other members of the nursing team, and perform these services in accordance with the physician’s orders.
Responsibilities
- Provide and coordinate quality patient care, maintain a full caseload, and meet productivity goals with accurate, timely documentation.
- Manage each patient’s care efficiently and cost-effectively to achieve positive patient outcomes.
- Oversee the overall plan of care and its outcomes, including care delivered by other nursing staff.
- Conduct admission and manage patient care by obtaining medical history, conducting physical examinations, evaluating patient/family needs and home situation, and determining the need for home health aide services.
- Interpret nursing and agency services to patients and families as part of care planning.
- Meet regulatory requirements and understand various insurance plans.
- Practice accepted nursing standards regarding infection control.
- Develop and implement the nursing care plan.
- Provide skilled nursing care including physician-ordered services, treatments, and preventive procedures.
- Initiate preventive and rehabilitative nursing procedures as appropriate for patient care and safety.
- Observe signs and symptoms, report reactions to treatments and changes in patient condition to the physician.
- Teach, supervise, and counsel patients and caregivers regarding nursing care needs and home-based problems.
- Oversee care given by Home Health Aides by supervising and evaluating their performance.
- Apply documentation according to standards of nursing practice.
- Utilize computer skills for accurate documentation.
- Document services rendered, note changes in patient condition/family/home situation, revise nursing care plans, record supervisory visits, evaluate patient care and progress, and close charts of discharged patients.
- Perform other duties as assigned.
Requirements
- Current, valid RN license issued by NYS Department of Education required.
- Minimum one year home care experience preferred.
- Knowledge of State and Federal regulations and payer guidelines.
- Ability to exert up to 50 pounds of force occasionally for medium work.
- Regularly expected to stand, sit, hear, walk, talk, stoop or kneel, and perform repetitive motion.
