About the Role
We are seeking experienced Case Managers for our new women's shelter in the Bronx. Case Managers are part of an interdisciplinary team focused on stabilizing shelter clients by strengthening independent living skills and supporting clients in achieving their housing goals. The successful candidate will have experience working with individuals experiencing homelessness and/or mental health challenges.
Responsibilities
- Conduct intake and assessments to orient clients to the program and develop individualized service plans based on identified needs and goals.
- Meet weekly with clients to support progress toward service plan goals.
- Coordinate services with the Housing Specialist, shelter staff, and external partners.
- Advocate for clients regarding benefits, entitlements, and service access.
- Provide crisis intervention and supportive counseling to strengthen clients’ decision-making and coping skills.
- Document all case management activities in accordance with agency and funding guidelines.
- Complete housing applications (both written and electronic) based on clients’ needs and eligibility.
- Achieve assigned monthly housing placement quotas.
- Facilitate workshops and groups, and escort clients to appointments as needed.
- Conduct weekly or biweekly unit or dorm inspections.
- Attend staff meetings and trainings to enhance job-related skills.
- Work flexible schedules, including evening and weekend shifts as required.
- Perform other related duties as assigned.
Requirements
- High school diploma, HSE, or GED required.
- Minimum of two (2) years of social services experience required.
- Valid Food Handler’s License or ability to obtain within six (6) months of hire required.
- Experience working with individuals experiencing homelessness, mental illness, substance use, and/or low-income populations.
- Strong relationship-building and communication skills required.
- Strong computer skills required, including Microsoft Word and Excel.
- Ability to work evenings and/or weekends on a rotating schedule.
- Prior experience working in a homeless shelter preferred.
- Knowledge of mental health, substance use, and homelessness service systems preferred.
- Bilingual English/Spanish preferred.
