About the Role
The Social Work Assistant will support unit social workers in providing services to patients and their families undergoing mental health treatment. As part of the interdisciplinary treatment team, this role is crucial in developing and implementing patient treatment and discharge plans, coordinating discharge planning for adult and child/adolescent inpatient psychiatry, and addressing psychosocial problems that may impede care. The role also involves developing interagency relationships, mobilizing community resources, and providing post-discharge telephonic support.
Responsibilities
- Rounds on identified Adult inpatient psychiatry patients prior to discharge for a warm handoff and review of the discharge plan via teach-back.
- Contacts discharged patients from Adult Inpatient Psychiatry telephonically for two weeks post-discharge to address barriers in the discharge plan.
- Assists the Unit Social Worker in coordinating the patient’s discharge plan, including referrals, continuity of care, safety planning, and documentation.
- Documents interventions in compliance with Division, Department, and Hospital Policy, including safety plans and continuity of care documentation.
- Attends Social Work Division and Department Staff meetings as required.
- Participates in Social Work Division activities, including timely completion of time reporting, forms, and mandatory training.
- Contributes to a culturally competent environment by treating patients, families, trainees, and co-workers sensitively, with attention to cultural differences.
- Participates in cultural competence events and training relevant to job duties.
- Provides social work assistant coverage and back-up during co-worker absences in inpatient services, as needed.
- Performs other duties as requested and assigned.
Requirements
- Bachelor's degree in Social Work required or equivalent combination of education and experience.
- Clinical or Community Social Work experience in a health setting preferred.
- Prior experience working in an acute psychiatric or behavioral health clinical setting preferred.
