About the Role

EAC Network is seeking a dedicated Case Manager for the Alternatives for Youth (AFY) Program to provide Home-based Crisis Intervention and case management services for client families. This role supports the Children & Youth Population and aims to make a real difference in their lives.

Responsibilities

Assist Spanish and English-speaking families in resolving crises and problems that led to a PINS petition.
Provide case management support to families in crisis struggling with substance abuse and mental illness.
Deliver intensive, home-based case management and crisis intervention to promote family stability.
Conduct assessments using the Child and Adult Needs Assessment (CANS) instrument.
Develop a Family Intervention Plan based on family needs using a strengths-based approach.
Make referrals to partnership agencies and outreach to new agencies.
Follow up with service providers to monitor family progress.
Assist parents and youth in learning behavior management techniques.
Maintain progress notes and accurate case records in the computer system.
Be on call to families contacting the AFY hotline as scheduled.
Transport and escort clients from appointments or to obtain emergency resources using personal vehicle when necessary.
Participate in team meetings, case conferences, and trainings.
Facilitate and develop monthly educational workshops for adolescents.
Participate in monthly recreational events for adolescents in the AFY program.
Hold weekly female and male adolescent groups.
Maintain progress notes and accurate case records with timely data entry in accordance with contract mandates.

Requirements

Bachelors Degree and one year of child and adolescent experience required or at least one-year community or social service work or child and adolescent experience.
Must have good communication skills, both oral and written in English and Spanish.
Must have valid New York State Drivers License and have access to a car.
Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software.
