About the Role
The Residential Case Manager provides case management and housing services to clients, facilitating their integration into the residence and community. This role ensures timely outreach and case management activities to meet program goals, including documentation in the program database and adherence to confidentiality regulations. The position involves ongoing assessment of client needs and collaborative development of service plans.
Responsibilities
- Provide case management and housing-related services to residential clients.
- Facilitate client adjustment and integration into the residence and community.
- Ensure timely flow of community outreach and case management activities.
- Enter assessment and progress note documentation into the program database.
- Conduct all electronic charting and reporting in accordance with confidentiality regulations and hospital policies.
- Conduct ongoing service need assessments with participants.
- Develop collaborative service plans quarterly addressing identified needs and goals.
- Maintain regular personal contact with clients.
- Promote trusting relationships to monitor physical and emotional capacity for independent living.
- Identify the need for additional services in the community.
Requirements
- At least one year of mental health experience with the target population.
- Experience in case management and/or residential program required.
- Valid driver's license in good standing.
- Knowledge of Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint) and database navigation required.
