About the Role
Provide tailored care and guidance to participants and families within the Assertive Community Treatment Program. Act as a member of a mobile, multi-disciplinary treatment team for individuals with severe and persistent mental illness, integrating information on supported employment, job development, and rehabilitation methods.
Responsibilities
- Provide tailored care and guidance to participants and families.
- Act as a member of a mobile, multi-disciplinary treatment team.
- Integrate information and expertise on supported employment, job development, and rehabilitation methods.
- Assist clients in developing skills in their environments of choice.
Requirements
- Master’s degree or higher in social work, psychology, mental health counseling, or marriage and family therapy.
- Rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred).
- A valid and clean NY State driver license.
- Minimum 1 year of experience in a treatment or rehabilitation setting.
- Fingerprinting, criminal record check.
- Approval from NYS Office of Mental Health.
