About the Role
Care Managers are the guardians of continuity of care, leading the full spectrum of patient care from pre-admission to discharge while ensuring quality outcomes. As a Care Coordinator, you'll serve as a highly valued and respected leader, empowered to make amazing things happen for our patients and their families.
Responsibilities
- Providing intensive care management for adult patients with complex medical needs.
- Serving as a strong and knowledgeable liaison between the interdisciplinary team of social workers, Physicians, Physical Therapists, Occupational Therapists, Cardiologists, and Intensivists.
- Directing all aspects of inpatient care.
- Developing individualized care plans.
- Ensuring optimal length of stay.
- Helping prevent hospital readmission.
- Anticipating outpatient service needs.
- Setting appointments and consultations prior to discharge.
Requirements
- Bachelors degree in Nursing (BSN)
- Current NYS Registered Nurse (RN) licensure
- 3 to 5 years of health care experience in an acute care setting
- Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical
- All employees should live within a commutable distance to NYP.

Salary/Compensations
- $134,900-$168,000/Annual
