About the Role
Provides counseling services including identifying, screening and assessing patients to determine the need for substance abuse services. Develops comprehensive treatment plans and provides case management, discharge planning, and continuing care services.
Responsibilities
- Identifying, screening, and assessing patients for substance abuse services.
- Identifying, screening, and assessing patients, families, and/or significant others for substance use disorders.
- Completing comprehensive assessments, including psychosocial.
- Providing ongoing orientation about the program and services.
- Developing comprehensive treatment plans collaboratively with patients and the interdisciplinary team.
- Supervising patient urine collection and monitoring substance use/abuse through urine toxicology.
- Providing comprehensive case management services, discharge planning, and continuing care services.
- Providing education and referrals to appropriate health and human services agencies.
- Completing all statistical reports as required by the hospital, department, and regulatory agencies.
- Providing community education in accordance with established hospital policy and Federal, State, and City standards.
- Participating in all related functions of the department.
Requirements
- Experience working with individuals diagnosed with mental illness and/or substance abuse disorders.
- Experience working in a substance abuse treatment setting.
- Computer literacy.
- CASAC required.
- High School Diploma or Equivalency.
- Undergraduate/Bachelor's degree.
