About the Role

The Clinical Nutrition Coordinator provides quality nutrition care and education to patients and family members, under the direction of the Director of Clinical Nutrition. This role coordinates departmental initiatives and education programs, maintains clinical expertise, and recommends appropriate nutrition therapy. The coordinator acts as a resource to clinical staff and provides feedback for their professional development, adhering to all regulatory guidelines and hospital policies.

Responsibilities

Demonstrates advanced practice in clinical focus area and seeks professional development opportunities.
Evaluates patient nutritional status using electronic medical records and interdisciplinary team input.
Applies knowledge of nutrition screening and assessment to provide medical nutrition therapy and develop individualized nutrition care plans.
Provides individualized counseling to patients and caregivers on food-drug interactions, therapeutic diets, and supplements.
Conducts nutrition-focused physical exams to determine malnutrition risk.
Documents all patient encounters in the electronic health record per policy.
Completes necessary paperwork for insurance billing and reimbursement.
Maintains administrative records, including productivity, competencies, and patient care documentation.
Functions as a vital part of the multidisciplinary team, engaging in discussions regarding patient nutrition care.
Participates in interdisciplinary rounds and care plan meetings as needed.
Coordinates projects and activities, including nutrition-related in-services, education programs, lectures, and external speaking events.
Assists with care of patients on home nutrition support.
Evaluates nutrition support prescriptions and communicates with the clinical care team.
Assists with discharge planning and hospital intake for patients on home nutrition support.
Demonstrates leadership through coordination and implementation of initiatives that promote nutrition services.
Coordinates quality assurance monitoring and reporting activities.
Offers outcomes-based resolutions that promote sustainable improvement and supports interdisciplinary performance improvement goals.
Develops and updates nutrition education materials for specialized patient populations.
Precepts dietetic interns, instructs them on competencies, develops learning modules, and completes student evaluations.
Participates in research activities for ambulatory practice.
Maintains a positive and professional demeanor and communicates effectively with patients, visitors, colleagues, staff, and supervisors.
Accepts and incorporates feedback for clinical development.
Supports department/program goals.
Adheres to department/hospital policies, quality standards, and safety standards.
Performs other duties as assigned.

Requirements

Bachelor’s degree or higher from an accredited institution.
Completion of ACEND-accredited dietetic internship.
3+ years of clinical experience.
Registered dietitian (RD) from the Commission on Dietetic Registration (CDR) required.
Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions required or process initiated upon hire.
Advanced Certification, i.e. Certified Diabetes Educator (CDE), Certified Nutrition Support Clinician (CNSC), and/or Pediatric/Renal/Oncology Specialty encouraged, but not required.
