About the Role

Responsible for assessing the risk and capacity of individuals referred by the community and addressing their needs.

Responsibilities

  • Conduct assessments to determine client risk and capacity.
  • Establish a comprehensive care plan for the client.
  • Provide information to meet client needs.
  • Make timely referrals and arrangements for services.
  • Follow up on all care plan items.
  • Ensure compliance with New York State Office of Children and Family Services and New York City Human Resources Agency contractual obligations and agency policies.
  • Advocate on behalf of the client.
  • Consult with supervisor regarding client needs and problems.
  • Provide crisis intervention to stabilize client situations.
  • Deliver cash, checks, food, mail, and other items to the client as needed.
  • Maintain monthly contact with clients.
  • Visit clients in their homes monthly to assess living conditions.
  • Participate in meetings to ensure client needs are addressed and coordinated.
  • Update involved individuals and agencies on client status.
  • Initiate and follow through on discharge planning.
  • Request, follow up on, and facilitate heavy duty cleanings.
  • Attend and participate in court hearings.
  • Prepare and maintain required documentation.
  • Complete reports, applications, and statistics thoroughly and accurately.
  • Meet deliverables by the due date.
  • Maintain a professional demeanor when communicating with clients, peers, supervisors, and collaterals.
  • Document all interactions in case notes.
  • Ensure client files are updated, accurate, and complete.
  • Participate in supervision and training as assigned.
  • Provide on-call services during out-of-office hours on a rotating basis.

Requirements

  • One year related experience and/or training.
  • Working knowledge of Microsoft Office Suite and other technology.
  • Excellent communication and listening skills.
  • Excellent customer service skills with a focus on treating clients with respect and dignity.
  • Able to multi-task and work independently with great attention to detail.
