About the Role

As a Clinician in the Youth ACT program, you will help children avoid hospitalizations and out-of-home placements by providing intensive, community-based services to youth and families who have not found success in traditional mental health and community treatment. You will report to the Director of Youth ACT and work on-site and in the community, traveling to client's homes.

Responsibilities

Provide intensive services to youth and families on a shared caseload with ample supervision and support.
Meet clients in the community, rather than in a clinical office setting.
Utilize specialized training in the Youth ACT model, person-centered planning, motivational interviewing, trauma-informed care, social determinants of health, cultural competence, and implicit bias.
Conduct risk assessments, substance abuse screenings/assessments, suicide assessments, mental health needs assessments, and social needs screenings.
Perform comprehensive psychosocial assessments to include diagnosis and needs for treatment plan development.
Conduct Child and Adolescent Strengths and Needs Assessments.
Collaborate closely with collateral providers involved in the lives of youth and families.
Provide holistic care through individual, group, and family counseling modalities.
Maintain flexibility in scheduling for community-based work.

Requirements

Master's degree required.
NYS professional licensure: LMSW, LCSW, LMHC, LCAT or LMFT.
One year post-graduate experience preferred.
NYS driver's license required for community-based travel.
