About the Role

Care Managers will bridge the gap between Adult Protective Services (APS) and community-based aging services. This role participates in a grant program designed to integrate aging services with APS, acting as a consultant and conducting joint assessments. The Care Manager will also lead the Care Manager group assigned to this project, facilitate referrals, and coordinate overall care management work.

Responsibilities

  • Provide care management to clients.
  • Accept referrals as assigned.
  • Complete comprehensive in-home assessments to determine needs.
  • Create and implement client-centered care plans in conjunction with APS.
  • Provide information and referral, advocacy, benefit and entitlement counseling.
  • Record case activity in the Lifespan database designated for the project.
  • Arrange mental health interventions for older adults and their caregivers, when necessary.
  • Conduct mental health screenings when appropriate.
  • Provide supportive counseling to older adults.
  • Work in tandem with APS to identify and provide linkage to appropriate ongoing mental health community services at intake.
  • Act as lead Care Manager for project activities and assist in coordination of other care management staff.
  • Participate in Community Care Connections team meetings and trainings.
  • Participate in Lifespan staff meetings and trainings.
  • Participate in quality assurance activities.
  • Provide input into the development of this pilot program.
  • Accept and seek guidance and clinical supervision.
  • Maintain up to date professional knowledge of the geriatric field, community services entitlement benefits, clinical skills and competencies.
  • Participate in training required for grant project activities including Motivational Interviewing, Trauma Informed Care and Racial Equity training.
  • Be aware of and actively support the Agency’s initiative of being collaborative and community focused, innovative and culturally competent.
  • Perform other duties as assigned by supervisor.

Requirements

  • Must be able to meet with clients in their homes.
  • A portion of the work is performed at a desk or at a personal computer station.
  • Verbal, written and telephone communication skills are required.
  • Strong written and computer skills needed.
  • Interface with all levels of personnel.
  • Ability to work in an Adult Protective Service environment and with older adult clients at risk of abuse, neglect, exploitation and self-neglect.
  • Ability to maintain composure under strict deadlines and difficult situations.
  • Ability to work in multi-cultural situations.

Experience Level

  • Two years in social work

Salary/Compensations

  • Hourly Rate: up to $25.00
