About the Role

The Senior Youth Care Coordinator oversees the operations of the Youth Care Management program, implementing and maintaining policies and procedures, and supervising a team of Youth Care Coordinators. This role involves managing a modified caseload and ensuring the smooth functioning of the program.

Responsibilities

  • Maintain all duties and responsibilities of a Youth Care Coordinator, including managing a modified caseload.
  • Supervise a team of 3 - 5 Youth Care Coordinators.
  • Review submission of all documentation in client charts for timeliness, content, and quality.
  • Maintain a working knowledge of team caseloads, Health Home policies and procedures, and systems.
  • Train new staff members in program policy, health home requirements, and document submission.
  • Attend intakes/home visits with team members to ensure staff safety, provide guidance, or offer consultation.
  • Act as a consultant for team members regarding community resources and social welfare programs.
  • Address client concerns and/or grievances that cannot be resolved directly with the Youth Care Coordinator.
  • Attend and participate in administrative meetings.
  • Attend individual supervision to discuss needs, concerns, and supervisory skills development.
  • Attend periodic trainings, webinars, and meetings to enhance skill levels and stay updated on system changes.
  • Co-facilitate monthly Youth Care Management meetings.
  • Obtain and maintain CANS-NY Assessment Certification (Supervisory staff must obtain an 80% or higher).
  • Implement and maintain program policies and procedures.
  • Responsible for revenue monitoring and reporting.
  • Implement and monitor Performance Improvement Plans.
  • Provide leadership with systemic and programmatic design.
  • Review all referrals and eligibility determinations, and assure maintenance of eligibility documentation.
  • Work closely with Program Coordinators to address staff issues and ensure smooth implementation of program policy.
  • Responsible for agency vehicles, including upkeep, documentation, and gas card.
  • Responsible for agency cell phone, laptop, and associated items.
  • Follow program guidelines as outlined in the personnel manual.
  • Report any issues and/or concerns to program administration.
  • Responsible for having access to an insured, personal vehicle.
  • Track and submit mileage.
  • Assist Program Coordinators with additional tasks concerning departmental needs.
  • Perform other duties as assigned.

Requirements

  • Licensed Masters-level Human Services professional preferred.
  • Experience working with youth and their families preferred.
  • Experience with the Health Home Serving Children program, Health Home systems, and care coordination preferred.
  • Strong and effective leadership skills, communication skills, and program development skills preferred.
  • Bachelor of Arts or Science Degree with two years of relevant experience OR a license as a Registered Nurse with two years of relevant experience OR a Masters with one year experience.
  • Must obtain and maintain CANS-NY Certification.
  • NYS driver’s license and access to a vehicle.
  • Excellent communication skills.
