About the Role
Coaches are needed to work with individuals experiencing homelessness to find rapid housing with private landlords. Coaches provide guidance, coaching, and support to vulnerable young adults at risk of homelessness, helping them achieve permanent housing with time-limited rental assistance. The role emphasizes a nonjudgmental, strengths-based, and collaborative approach, ensuring youth preferences guide service plans. Coaches will collaborate with their team to equip youth for independent living, including crisis intervention, entitlements procurement, healthcare, mental health, independent living skills, job/career assistance, financial literacy, recreational opportunities, and education.
Responsibilities
- Completes the intake process with youth according to established guidelines.
- Makes appropriate contact with and/or referrals to other agencies.
- Completes needs assessments, including mental health and social history.
- Obtains legal documents (birth certificates, NYS Identification, social security).
- Assists with housing applications, employment, and budgeting.
- Enrolls youth in school where applicable.
- Facilitates, develops, and participates in skill-building groups.
- Provides bi-weekly individual coaching to each youth.
- Makes referrals to outside services when needed (e.g., substance abuse, mental health).
- Maintains updated chart records for all current and past residents.
- Completes all mandatory paperwork required by contracts and funding sources.
- Works alongside staff to develop and evaluate programming.
- Conducts outreach presentations as needed and assigned.
- Conducts monthly home visits in accordance with the CTI Model.
- Attends regular staff meetings and trainings to support youth needs.
- Uploads support plans and relevant documentation reflecting CTI goals and stages.
- Maintains an active caseload of 15+ tenants.
- Manages tenant referrals and follows up with phone calls.
- Cultivates and fosters working partnerships with community landlords.
- Prepares and tracks documentation, including tenant services, via the Eccovia Data System.
- Participates in in-service trainings and on-site workshops.
- Attends external trainings and workshops at the direction of management.
- Performs other job-related duties as assigned.
Requirements
- Experience assisting with obtaining and maintaining government benefits.
- Experience accessing services.
- Experience managing medication.
- Comfort running group meetings when necessary.
- Knowledge of group dynamics and interaction, especially with adolescent populations, is preferred.
- Ability to act responsibly in stressful/crisis situations.
- Written and verbal communication skills sufficient to effectively provide direction and counseling to clients and maintain required documentation.
- Must be capable of and willing to make home visits unassisted throughout the greater New York area.
- Must ensure a realistic and secure approach to home visits.
- Must be capable of independently and professionally assessing the physical and emotional well-being of the client.
- Must be capable of ascertaining the adequacy of the physical environment and treatment/educational/vocational programs.
- Ability to independently organize time and prioritize responsibilities to plan and carry out a demanding schedule.
- Valid New York State driver's license, to provide transportation when required and to handle certain crises.
- Previous experience in the social services field or supportive housing is preferred.
