About the Role

St. John's Riverside Hospital is seeking a dedicated professional to join their team. This role involves developing and educating new departmental employees, managing daily operations, and ensuring comprehensive discharge plans for patients. The ideal candidate will possess strong assessment, collaboration, and communication skills to support patients and families through their healthcare journey.

Responsibilities

  • Participate in the development and education of new departmental employees.
  • Educate new employees on departmental vision, mission, function, theoretical framework, and goals.
  • Ensure new employees understand departmental policies and procedures.
  • Perform monthly staff meeting prep, including agenda and deliverables.
  • Maintain staff meeting attendance logs and ensure team attendance at educational meetings.
  • Manage and lead the bed board meeting daily.
  • Collect and share information with designated department staff.
  • Assist in meeting the psychosocial needs of patients and families through assessment.
  • Screen patients for continuing care needs.
  • Educate, coordinate, and collaborate with healthcare team members on discharge plans.
  • Ensure appropriate use of resources within an appropriate length of stay.
  • Collaborate with the multidisciplinary team on the patient’s treatment plan.
  • Review admissions and continued stays in accordance with establishing discharge plans.
  • Provide supervision and guidance to Continuing Care Coordinator staff.
  • Elevate staff performance issues and monitor employee tasks for compliance.
  • Complete Psychosocial assessments within three days of patient’s admission.
  • Collaborate, coordinate, document, and manage discharge planning needs.
  • Review cases for quality of care and utilization management issues and escalate unresolved issues.
  • Provide supportive care and referrals for patients with terminal diagnoses, coping difficulties, and caretaker fatigue.
  • Assess patients in Maternity and ER through consults and screen for high-risk criteria.
  • Inform patients and caretakers regarding admission criteria and referral processes.
  • Follow through on referrals to ensure safe and timely discharge plans.
  • Counsel and support patients and their caretakers regarding anxieties and stress related to illness and hospitalization.
  • Collaborate with community resources to develop discharge plans and facilitate continuity of care.
  • Maintain an up-to-date resource file and follow referral procedures.
  • Utilize assessment skills to determine patient’s discharge planning goals, Health Care Proxy and Caretakers discharge concerns, and need for institutional/specialized care.
  • Incorporate multi-disciplinary team plans of care into appropriate discharge plans.
  • Assist in obtaining MD orders and insurance authorization for post-hospital needs.
  • Provide coverage for Dobbs Ferry, including pre-assessment and facilitating Joint Class.
  • Order equipment for patients admitted through Same Day Surgery preparing for total joint replacement.
  • Be assigned to the Ambulatory Services Unit for discharge planning needs when consulted.
  • Collaborate closely with the hospital benefits area in identifying change of benefit status or lack of insurance.
  • Assist with complex discharge planning for patients with complex medical, psychosocial, and resource challenges.
  • Participate in performance improvement activities by monitoring length of stay and timely documentation.
  • Escalate cases regarding quality of care and utilization issues to the appropriate administrator.
  • Identify systemic issues, participate in department/hospital committees, and recommend improvements to continuing care processes, policies, and resource utilization.
  • Responsible for the daily schedule and ensuring adequate coverage.
  • Orient newly hired employees and complete required performance checklists.
  • Provide performance-enhancing trainings and education to the department.
  • Guide peers pursuing their LCSW on the completion of NYS application and submission for licensure.
  • Assist the departmental director in creating the monthly staff meeting agenda, presenting deliverables, and maintaining attendance.
  • Assist in the interviewing of potential new departmental candidates.
  • Assume other responsibilities when assigned.

Requirements

  • New York State Screen assessment certification desirable.
  • Able to read, write, speak, and understand the English language.
  • Ability to communicate Spanish or community dominant language is an asset.
  • Effective professional rapport with physicians, patient, family/visitor, peers and supervisors.
  • Demonstrates sensitivity and compassion.
  • Knowledge of community services, health, welfare, and social agencies.
  • Demonstrates flexibility and creativity.
  • Able to function well under pressure.
  • Good leadership ability and good judgment.
  • Good mental and physical health.
  • Must be educated in the use of the particulate respirator (mask).
  • Masters degree required.
  • At least 3 years discharge planning or Case Management experience.
  • NYS license required LCSW.
