About the Role
Pulse is seeking an experienced Band 7 Clinical Anticoagulation Pharmacist for a locum position in Chertsey, Surrey. This role involves providing specialist support to the pharmacy team, optimizing anticoagulation therapy, and ensuring high standards of patient care within an NHS hospital.
Responsibilities
- Provide specialist clinical pharmacy support for anticoagulation services.
- Review and optimize anticoagulation therapy for safe and effective patient care.
- Offer expert pharmaceutical advice to healthcare professionals.
- Conduct clinical medication reviews and reconciliation.
- Monitor patients on anticoagulant therapy and support dose adjustments.
- Contribute to the development of anticoagulation guidelines and protocols.
- Support patient education on anticoagulant medicines.
- Participate in audits, governance, and quality improvement initiatives for anticoagulation services.
Requirements
- Registered with the General Pharmaceutical Council (GPhC).
- Previous NHS hospital pharmacy experience at Band 7 level or equivalent.
- Experience working within anticoagulation services or managing anticoagulant therapies.
- Strong clinical knowledge of anticoagulant medicines and monitoring.
- Ability to work independently and within multidisciplinary teams.
- Excellent communication, organizational, and clinical decision-making skills.
- Commitment to delivering high-quality, patient-centred care.
