About the Role
The Case Manager is responsible for providing case management services to clients in transitional housing, managing a caseload of 25 clients, and ensuring the integration of housing and employment goals while complying with funder requirements.
Responsibilities
- Utilize evidence-based practices (E.B.P.), Motivational Interviewing (MI), Trauma-Informed Care (T.I.C.), critical time intervention, and boundary setting to assist clients in achieving their goals.
- Complete initial client assessments, including Case Assessment Form, Initial Service Plan, and intake documentation, within 48 hours of arrival.
- Initiate referrals for emergency services.
- Complete intake information and CARES data entry in the Uniform Client Chart.
- Ensure CARES data and supporting documents are printed and filed correctly.
- Conduct client orientations covering facility rules, rights, responsibilities, grievance process, and emergency procedures.
- Assist clients with transitioning to residential units and provide housekeeping items.
- Provide comprehensive case management, including crisis intervention and counseling.
- Develop and manage Independent Living Plans (I.L.P.s) for each client, tracking progress and ensuring compliance.
- Offer hands-on training and coaching to help clients achieve I.L.P. goals.
- Ensure clients understand and meet their responsibilities as outlined in the Client Acknowledgment Responsibility Form (C.A.R.F.).
- Maintain accurate and current client data in the Uniform Client Chart.
- Assist clients in obtaining income and public benefits.
- Refer clients to appropriate medical, mental health, educational, job training, legal, and community services.
- Track referrals and monitor client progress.
- Assess client job readiness.
- Ensure timely and accurate input of client information in the Uniform Client Chart, complying with regulatory requirements.
- Collaborate with Housing Coordinators to ensure clients are compliant in seeking permanent housing.
- Attend required meetings and staff training.
- Work evenings, one Saturday per month, and holidays as scheduled.
- Prepare mandated reports as required by supervisors, contracts, and funding agencies.
- Conduct weekly client unit inspections and review findings.
- Perform other related duties as needed.
- Ascend/descend stairs in the assigned building.
- Escort clients to appointments, including apartment viewings.
Requirements
- Commitment to H.O.G.A.R.'s mission, vision, and values.
- Bachelor's degree in social work or a related field.
- One (1) year of social service experience or approved equivalent.
- Familiarity with E.B.P., such as strength-based case management, motivational interviewing, trauma-informed care, critical time intervention, boundary setting, and cultural reflexivity preferred.
- Familiarity with entitlement systems and procedures preferred.
- Experience with standardized assessment tools preferred.
- Excellent organizational, written, and verbal communication skills preferred.
- Ability to work effectively in a team environment.
- Computer skills and knowledge of CARES are a plus.
- Bilingual – English/Spanish proficiency is a plus.
